2026-2027 Athletic Physicals

Sports Medicine Associates of San Antonio will be at Bandera Middle School on May 14 to administer athletic physicals for the 2026-2027 school year. As a reminder, BISD requires every athlete grades 7-12 to have a new physical at the beginning of each school year. Consent forms will be sent home with all athletes and they are also available on the athletics website (https://sites.google.com/banderaisd2.net/bisdathleticsweareone/home). Consent forms must be turned in by May 7. The cost is $20 per athlete and no cash or checks will be accepted.

These form will be sent home or are available for download on the athletic website (The consent form allows for the doctors to administer the physical without a parent being present. The August Heart form is for an ECG screen that all athletes are required to get per Sports Medicine Associates of San Antonio. The Medical History form must be filled out completely and signed by the parent or guardian.
